To improve care for older persons with mental health issues wherever they may reside.
The Community Outreach Team is a service administered through the Geriatric Psychiatry Program of St. Joseph's Care Group and funded by the Ministry of Health and Long Term Care.
Our focus is assisting with the management of clients with behavioural issues. Clients who are transferred to long-term care facilities are also followed by their case manager to assist with the transition. The program has evolved and is providing outreach services to the broader community.

Individuals over the 65 years of age or adults who are experiencing mental health difficulties usually attributed to aging.
- A community-based interdisciplinary case management approach with active participation of the client & family
- Our team consists of: Manager, Team Leader, Case Managers (interdisciplinary), clerical support
The Community Outreach Team provides non-medical intervention and behaviour strategies that include:
- Assessment and management of associated issues such as functional dependence, safety, caregiver stress, education, community services and future planning

Recommendations for prevention, rehabilitation and behaviour management

Advocacy

Working with systemic issues to enhance appropriate support to assist the client

Consultation may lead to a referral to an affiliated in-patient or out-patient geriatric service

Clients of the program who transfer to long term care receive team support to assist them and their caregivers as they adjust to the new facility.
