1. Why would I need this service?
If you require rehabilitation to improve your mobility, personal care and overall function.
2. How would I qualify for this service?
You must be:
- 60 years old, or older.
- able to tolerate a minimum of 30 minutes of therapy, twice per day, 5 days/week.
- be medically stable.
- require moderate to maximum assistance with activities of daily living.
- cognitively able to participate and follow instructions, although may require repetition or have delayed response.
- be motivated to participate in therapy.
3. How I can get started with this service?
You need a doctor’s referral. Most clients are referred to this service after being admitted to the Thunder Bay regional Health Sciences centre for a medical occurrence or from an other program in St. Joseph’s Care Group. However. you can be referred by your physician from the community.
4. When can I take this service?
If you are coming from the Thunder Bay Regional Health Science Centre, the admission facilitator will determine if you are an appropriate candidate for the program. If you are referred by your physician from the community, the social worker and a physiotherapist or an occupational therapist from the react team will assess you in an outpatient clinic at St. Joseph’s Hospital to determine if you are an appropriate candidate.
5. What will this service offer?
You will receive 2-6 weeks of a slow rehabilitation program provided by an interprofessional team which includes:
- Medical care by a physician.
- Nursing rehab care.
- Occupational Therapy.
- Physiotherapy.
- Social Worker.
- Spiritual Care.
- Speech therapy.
- Dietician consultation.
- Respiratory therapy.
- Discharge planning.
- Home visits.
- Family conferences.
6. What is the average length of stay?
The average length of stay of a client is 5 weeks.
7. How is my discharge determined?
When the react team determines that you are not making anymore significant improvements and/or your needs can be met in the community or a long term care home. This information will be discussed with you and your family ahead of time to allow for discharge planning.
