Geriatric Assessment Program
The Geriatric Outreach Program provides a comprehensive assessment by a Geriatrician and Nurse Practitioner. This comprehensive Geriatric Assessment Program is the only program of its kind in the Northwest Region. Referrals must be made by a physician. A detailed home assessment will be completed by the Nurse Practitioner. The assessment focuses on the clients medical, functional and cognitive needs in order to optimize their well-being. Assessment with the Geriatrician will occur following the Nurse Practitioner’s visit.
