Type 1 & Type 2 Diabetes in Adults

Who is this program for?

Adults who have been diagnosed with Type 1 or 2 diabetes are served in this program. We see people in Thunder Bay but also some with special requirements such as insulin infusion pumps will be seen from the region of Northwestern Ontario.

Why would I need this program?

Diabetes management can be difficult if not impossible without the right knowledge tools and resources. Poor diabetes management over many years results in complications of diabetes. These can be prevented, delayed and/or halted with good diabetes control. We provide education and support as well as access to the right tools and resources to help you in your goal of self management.

What is this program about?

Services for adults include overall diabetes education as well as services for intensive diabetes management, Insulin Infusion pump therapy and meal planning both basic and advanced including carbohydrate counting. We are a designated Assistive Devices Program registered for the provision of insulin infusion pump training in Northwestern Ontario. All services for diabetes education in self management are directed toward the needs of the individual as evaluated after a complete assessment by a nurse and a dietitian.

Where is this program offered?

We provide diabetes education services to the different segments of our adult community. This includes providing services at different sites in the community such as 55+ Center, Herb Carroll, Suomi Koti, PR Cook Apartments, etc. As well as our offices.

How is this service provided?

All people with Type 1 or 2 diabetes are seen individually with a team of a nurse and dietitians to assess where you are in diabetes control and develop a plan for where to go. The team then decides how often they need to see you and follow-up is provided. Within this program we offer Intensive Management of diabetes and Insulin Infusion pump training and management. We are the designated ADP insulin pump site for Northwestern Ontario. Most of these services are provided out of our office however some work is done by phone or email. We also provide a drop in service for those who have completed the program but wish to come back with questions. The dates and times of these are provided to those who are encouraged to attend this service.

How do I get an appointment?

Most people are referred by a doctor; however we do accept referrals from other health care providers or even self referrals. Once the referral is received you will be contacted by phone or letter in regards to setting up the first appointment. After the first appointment follow-up appointments will be made in the office.
