Quality Improvement Plan (QIP)

Quality improvement is an ongoing priority that helps us continually find new and better ways of doing things so that we can enhance care for our clients, increase satisfaction and achieve even better clinical outcomes.

Our Quality Improvement Plan (QIP) is one tool that we are using to help us document and review our current performance in a variety of areas. With this plan, we will be able to very clearly see our targeted areas for improvement and chart our progress.

In June 2010, the Ontario Government passed the Excellent Care for All Act. This legislation will help support hospitals to further improve the quality and safety of care they provide for members of our community.

One of the ways that the Excellent Care for All Act is helping hospitals meet our community's expectations regarding quality, client safety and accountability is through the public reporting of Quality Improvement Plans (QIP).

Quality Improvement Plans provide a meaningful way for St. Joseph's Care Group to clearly articulate our accountability to our community, clients and staff. Our QIP is focused on creating a positive client experience and delivering high quality health care.

How to Read the Quality Improvement Plan

Our Quality Improvement Plan (QIP) is made up of two parts, Overview and Improvement Targets & Initiatives:

A document that provides a brief overview of our quality improvement plan, highlighting and listing St. Joseph's Hospital and Lakehead Psychiatric Hospital's top priorities for the year and how Executive Compensation is linked to the achievement of performance improvement targets outlined in our Quality Improvement Plan.

Improvement Targets & Initiatives (Part B)

A spreadsheet that includes our improvement targets and initiatives. The spreadsheet includes a core set of indicators that all similar hospitals across the province are working on. The Ontario Health Quality Council has requested that all hospitals report on a series of core indicators to support province-wide comparisons. The core indicators that apply to our hospitals are reflected in our QIP. These core indicators include reduce clostridium difficile associated diseases, improve hand hygiene compliance before client contact, improve organizational financial health, and improve client satisfaction.

Progress on QIP Year One 2011 - 2012

The Report on Progress is an opportunity for St. Joseph's Care Group - Lakehead Psychiatric Hospital and St. Joseph's Hospital, to demonstrate to our stakeholders the progress we have made to date on our priority indicators. It is an opportunity for us to describe our quality improvement journey and identify and discuss some of the challenges we faced in attaining the goals set out in our Year One Quality Improvement Plan (QIP).

The Report on Progress requires that we report our "Current Performance" according to certain prescribed data reporting periods which do not necessarily reflect the most recent, measured impact of our quality improvement initiatives. Therefore, St. Joseph's Care Group will be reposting the report on Year One QIP progress that is truly reflective of the entire Year One QIP results, in June 2012.
