St. Joseph's Care Group recognizes you, our client, as a unique individual with distinct wants and needs. To ensure that you, your family members and supporters are treated with dignity and respect and in a manner that is courteous, fair and considerate, St. Joseph's Care Group believes that you have the right to:
- Access programs that reflect your physical, spiritual, social, emotional, vocational, educational and recreational needs
- Receive equitable access to care regardless of your age, colour, creed, cultural background, sexual orientation, religion, financial or medical status
- Be treated with courtesy and respect in a way that fully recognizes your dignity and individuality
- Receive care in an environment that supports your mental, physical, and spiritual needs
- Be informed of your medical condition, treatment and proposed course of action
- Give or refuse consent to treatment, including medication
- Have your questions regarding treatment answered
- Be informed of the consequences of giving or refusing consent
- Have the opportunity to participate fully in making any decision regarding your care
- Be assured that personal, financial, medical and other records will be kept confidential and made available only to authorized persons within the organization or be informed of those individuals who have access to the records
- Have access to information contained in your health record and have your condition, care and treatment explained in easily understood terms
- Be kept informed about rules and regulations governing the facility
- Be kept informed about services provided at an extra cost
- Be afforded privacy during all forms of treatment
- Have research adequately explained prior to considering the provision of consent
- Consent, refuse consent, or withdraw consent to participate in research activities
- Exercise the rights of a citizen to raise concerns or complaints regarding services or supports on behalf of yourself or others and be assured that concerns will be addressed in a timely manner
- Have the right to notify health care providers or Administration of any concerns or complaints regarding services or supports
- Have personal needs met in accordance with the program(s) mandate
